Application Criteria
To attend the 18th Annual Interventional Fellows Course, you must complete the application and submit it for consideration to the Cardiovascular Research Foundation, the course sponsor. Once received, the application will be reviewed by the course organizers.
The Interventional Cardiology Fellows Course can accommodate up to 180 fellows. For an application to be considered, the applicant must be included in one of the following categories:
- An interventional cardiology fellow in an ACGME accredited program (US only). These applicants will be granted first priority if the application is received on or before February 29, 2012; after that date, a spot in the course cannot be guaranteed and applications will be accepted on a first-come, first-served basis depending upon availability.
- An interventional fellow in a non-ACGME accredited program (US only). These applicants will be granted 2nd tier priority if the application is received on of before February 29, 2012. After that date, a spot in the course cannot be guaranteed and applications will be accepted on a first-come, first-served basis depending upon availability.
- A fellow in a 4th year cardiovascular disease fellowship program (US only). Applications will be processed after March 5, 2012 on a first-come, first-served basis, depending on availability.
- A fellow in a 3rd year cardiovascular disease fellowship program (US only). Applications will be processed after March 9, 2012 on a first-come, first-served basis, depending on availability.
Note: 1st and 2nd year cardiovascular disease fellows may attend on a paid basis.
For those fellows accepted into the course, the Cardiovascular Research Foundation will pay coach air travel and 4 nights of housing at the Loews Miami Beach Hotel (room and tax only). There is no registration fee to attend the course. Ground transportation, meals outside of course functions, guests travel/expenses and other miscellaneous expenses are not reimbursable and are the responsibility of the attendee.

Explanation of Application Process
What happens after my application is submitted?
You will receive a confirmation email containing a form for your Fellowship Program Director’s Endorsement. You can either fax or email this form to the Cardiovascular Research Foundation.
Note: You will not be accepted into the program without this signed form.
My application has been accepted. What happens next?
You will receive an acceptance confirmation letter via email. This letter will include information about selecting industry supported workshops, housing details and travel arrangements.
Note: Please do not contact CRF to confirm that your Director's Form has been received. You will be contacted if we have not received your form.
My application has not been accepted. Are there other options available to attend this course?

What is the meeting cancellation policy?
In event of the need to cancel, please notify the Cardiovascular Research Foundation no later than April 12, 2012. Fellows who cancel their attendance at the course will not be reimbursed for any expenses including travel or hotel cancellation charges. Please see below for hotel cancellation information.
Housing at the Loews Miami Beach Hotel
Each extra person sharing a room (beyond 2 adults) will be charged an additional $30.00 plus tax, per night. There is no charge for children up to and including the age of 18 years who share with their parents. Maximum occupancy per room is three adults per king bedded room and four people per double bedded room.
Housing Cancellation Policy: If you have housing at the Loews Miami Beach Hotel, your reservation must be cancelled within the policy of the hotel, or your credit card will be charged for up to a four night stay. This charge will not be reimbursed by the Cardiovascular Research Foundation. You must contact Loews Miami Beach Hotel directly at 877-LOEWS-MB to cancel your reservation.
Housing
Housing (room and tax only) is paid by the course sponsor for fellows whose applications have been accepted. Fellows attending the course must arrive at Loews Miami Beach Hotel on Thursday, April 19th by 12 noon. You will receive a credit card form upon acceptance into the course that must be completed to guarantee your housing. In the event of a late cancellation, a no-show, or an early departure, your credit card will be charged for up to a four night stay.
Travel Arrangements
Air travel arrangements are made through AMEX Business Travel. The credit card provided will be used by AMEX Business Travel to book your airfare. The charge will not be made to your credit card until you have approved the itinerary. You will be reimbursed the cost of the airfare after attending the full meeting and submitting a reimbursement form.
